    Office Policies

    Booking

    To secure your booking, patients are required to pay a $100 deposit on all appointments with Dr. Qazi. This deposit will be used towards your treatment.

    Cancellations

    In regards to cancellations, appointments may be rescheduled or cancelled for a deposit refund if done 24 hours prior to your scheduled date and time. Any cancellation made after this period is subject to a $50 fee and the deposit will be kept as a result.

    Late Arrival

    As a courtesy to others, if a patient is more than 15 minutes late, they may have to be rescheduled and the deposit will be kept as a penalty.

    Payments

    All Payments must be made in full at the time of treatment. If payments are not made, an additional $50 will be charged to your bill weekly and your account may go to collections.
