ALL TREATMENTS MUST BE PAID FOR IN FULL PRIOR TO BEING SERVICED.

Booking A Consultation:

A $100 deposit is required at the time of booking to secure a consultation appointment. This deposit can be used towards services with that provider within a year of purchase. A refund of this deposit may be issued only if the appointment is canceled before 48 hours. Deposits are kept as a fee if appointments are canceled within 48 hours or if a patient misses their appointment without notice. No exceptions.

Booking a Non-Surgical Procedure:

A 10% deposit or a minimum of $100 for procedures under $1000 is required. A refund of this deposit may be issued only if the appointment is canceled before 48 hours. Deposits are kept as a fee if appointments are canceled within 48 hours or if a patient misses the appointment without prior notice. No exceptions.

Booking a Resurfacing Laser or a Surgery:

A non- refundable $1000 deposit is required at the time of booking and a full payment is due fourteen (14) days prior to your scheduled procedure/ surgery. Failure to make a full payment by that date will result in a canceled procedure/ surgery. Lab requisition, preoperative clearances, garments, and prescription medications may be required and are not included in procedure fees. It is the patient’s responsibility to complete those requirements if advised by your provider. If those requirements are not met, your procedure will be canceled. Cancellations made prior to fourteen (14) days may receive a refund of any payment made beyond the $1000 deposit. Cancellations made between fourteen (14) days and seven (7) days prior to your procedure will result in a 50% or minimum of $1000 cancellation fee. Cancellations made within seven (7) days prior to your procedure day will result in a 100% cancellation fee. Surgery deposits may not be transferred towards any other non surgical service. Deposits and any paid services expire after six months if a patient does not perform their procedure for any reason. Credits do not remain on file indefinitely. It is the patient’s sole responsibility to make sure they are healthy for surgery before consulting and scheduling their surgery. If you are not cleared for surgery for any reason including medical conditions or pregnancy, the same rules and cancellation fee structure apply. No exceptions.

Payment Methods and Financing Options:

Payments may be made in the form of Cash, Debit, Credit, Visa, Mastercard, Discovery, or American Express. Financing options such as Care Credit and Alphaeon Credit may be acceptable with varying options of 6 month, 12 month, or 24 month interest free payments. Financing plans depend on the purchase price. Please ask a staff member about which option you may qualify for. Health insurance is not accepted.

Prepaid Packages:

When applicable, packages must be paid in full prior to the first session being serviced to benefit from the special pricing. No further deposits are necessary to schedule fully paid packages. If no notice is given and a patient misses their prepaid service, they then forfeit the service scheduled and cannot reschedule without a new deposit. If the patient requests termination of future procedures from a prepaid package, the refunded amount will be determined based on a pay as you go rate for the already serviced items. Prepaid procedures may not be transferred to any other procedure. All services in the package must be used within one year of the purchase date and any remaining services will be forfeited and expire after one year. Credits do not remain on file indefinitely. No exceptions.

Late Arrivals:

Your time and our provider’s time is important to us. To prevent patients after you from waiting for their scheduled appointment, if you are more than 15 minutes late, you will be required to reschedule. Your deposit will be kept as a fee and a new deposit will be required to reschedule. No exceptions.

Refunds:

When applicable, all cash payments will be refunded to you in the form of a check within 30 days. All other refunds will be issued in the same form as the payment was received.

Prepaid Services Expiry:

Any non-surgical treatments which have been paid for and not serviced will be forfeited and expire after one year from the purchase date. Credits do not remain on file indefinitely. No exceptions.

Products:

Products, excluding prescriptions, can be exchanged or returned within 30 days of purchasing. No exceptions.
